What happens after the review meeting?

Copies of the Transition Plan should be sent to the young person and their parents or carers and the other people present at the meeting.

Year 9 reviews are the start of a longer process for making decisions about a young person’s future. For it to be effective, it’s essential that the Transition Plan is flexible enough to be adapted and developed in future reviews.

Who is responsible for carrying out the Transition Plan?

The Connexions service is responsible for overseeing the delivery of the Transition Plan and the Connexions Personal Adviser (PA) should co-ordinate its delivery (SEN Code of Practice, 2001).

Parents suggest that ongoing support from the relevant agencies is important to making the Transition Plan work successfully. Frequent visits from professionals, readily available information and ‘buddying’ in school can all help.
