There are various ways to integrate person centred planning into the transition planning process for a young person with autism. One approach is to focus on three questions:
The year 9 person centred transition review focuses on what’s important to the young person, what support they need and what is and isn’t working in their life.
The year 10 person review process takes this further by looking at what is possible, taking into account positive examples of what other people have achieved. It will prioritise the possibilities the young person wants to focus on.
The ideal is that the transition planning meetings will help young people to discover and create the futures they want. As a result, we hope fewer and fewer people will go into traditional day services. Instead we expect to see more young people with autism in paid work, with their own individual budgets or direct payments and employing their own staff. People with autism should be able to enjoy full lives and be contributing members of their communities.
